Under the direction of the Nursing Coordinator, or designate, the LPN is responsible for the care of residents in keeping with the philosophy and policies of Extendicare and carrying out nursing, resident care, and administrative duties in the Nursing Department as directed. Utilizing a primary model of care the LPN will provide resident care which includes assessing and developing care plans; monitoring; measuring outcomes; reporting; updating professional competence; participating in client‑focused teams with other healthcare providers. The LPN advocates for clients, communicates client needs to healthcare stakeholders, and reports on abuse or unsafe conditions. Additionally, the LPN educates and trains residents and families on health care issues, teaches residents and families to use assistive devices, and raises awareness of community services.
