Job Summary
The WISe (Wraparound with Intensive Services) Care Coordinator is responsible for leading the youth and family teams through the wraparound process. The care coordinator uses strong leadership skills to organize, develop and facilitate wraparound family team meetings, and is responsible for leading the team through the phases of WISe.
