Program : Community Outreach & Care Coordination
Type: Full-time (Non-exempt)
Pay Range: $22.06 - $29.69 per hour
Schedule : Monday - Friday; 9am-5pm w/flexibility
Notes : Local travel; a NYS Driver’s License and personal vehicle is required
