Job Description
As a Care Partner, you’ll guide customers through their care journey with compassion. Working closely with families, clinical teams, and external providers, you’ll develop holistic health plans, coordinate services, and ensure customers receive safe, person-centred care that enables them to age well at home or in a place of their choosing.
About the role: Contract: Permanent Full-Time
Hours: Work either 8:30am - 4:30pm or 9am - 5pm Monday to Friday
Hourly rate: $44.23p/h + Super
Office location: Hybrid - work from home 2 days and 3 days in our Cairns office
Service region: Cairns and surrounding areas
