Support our OT clinical team helping SNF residents rebuild skills for daily living and independence. You'll work under OT supervision implementing individualized treatment plans in a team-driven environment.

Responsibilities:

• Implement OT Plans of Care under the direction and supervision of a licensed Occupational Therapist
• Provide ADL retraining, fine motor activities, cognitive exercises, and UE strengthening as directed
• Maintain timely daily documentation in compliance with Medicare, Medicaid, and facility requirements
• Communicate resident progress and any status changes to the supervising OT in a timely manner
• Participate in IDT meetings and assist with adaptive equipment recommendations and discharge planning
