Excel as a Chronic Disease Management Nurse at Eston Health Centre, providing patient-centered care in a part-time role with flexible shifting. This position focuses on collaborative care.
As part of the Chronic Resident Unit team, your main focus will be on coordinating and delivering quality nursing services to patients facing chronic health conditions.
