We have a Relief Full-Time opportunity for a Clinical Pharmacist at Lions Gate Hospital in North Vancouver, BC, available until approximately December 2027 or until the return of the incumbent.

Responsibilities
- Consult with physicians, nurses, and other healthcare providers to develop and discuss therapeutic plans for each patient based on chart review and assessment of clinical responses.
- Ensure appropriate documentation is maintained in the patient health record in accordance with regulations, protocols, and practice standards.
- Individualize patient medication therapy by reviewing literature, patient charts, lab results, and consulting with the patient and healthcare team.
