This position is located in the Muskoka region (Huntsville, Bracebridge and Gravenhurst), and will require travel around the North Simcoe Muskoka Region.
KEY ACCOUNTABILITIES
- Provide comprehensive geriatric assessment, consultation and management, with a specific focus on serious mental illness and/or responsive behaviours through outreach and virtual care delivery.
- Work collaboratively with patients, caregivers, other members of the NSM SGS interdisciplinary team, other health care providers and relevant partners to support the needs of the patients and their caregivers and communicate across the health system.
