Job Description
KEY FUNCTION
To provide comprehensive nutritional assessments and care to complex medical patients, with a specific focus on the older population.
REPORTING
Under the general direction of the Clinical Manager, Acute and Reactivation Care Unit.
DUTIES
- Interpret anthropometric, biochemical, physiological data in relation to nutrition status.
- Calculate patient’s daily nutrition requirements (macro- and micro-nutrients including energy, protein, vitamins, minerals, fluids, etc.) based on available data.
- Provide Enteral and Parenteral Nutrition support, lab interpretation and correction.
- Provide therapeutic counselling to patients and/or family/caregiver(s) on an individual basis.
- Monitor patient progress and liaise with the multidisciplinary treatment team and other relevant health agencies (i.e. Community Care Access Centre).
- Complete nutritional supplement prescriptions as required...
