Role Overview
The Discharge Planner reports to the Manager, Inpatient Surgical Care/Child Life Services and provides leadership in planning and coordinating the timely discharge of patients from the hospital, including authorization of NSH Continuing Care services. The role embraces the Home First philosophy and delivers hospital‑based care coordination assessments, resource allocation and authorization of continuing care programs to support smooth transitions from hospital to home across all NSH zones.
