Location: #45905 Yale Rd, Chilliwack BC V2P 2M6
Schedule: Working hours are 9:30am - 5:30pm PST, and 1 remote day (assigned by Centre Manager)
Travel Expectations: Ability to work in different locations in the catchment
What you’ll do
- Conduct Client Needs Assessments with eligible clients as the first step in their individualized action plan
- Collaborate with clients to develop, update and revise action plans, in line with policy
- Overall responsibility for the management of assigned client case files in the primary information system (ICM), including documentation in client eligibility determination, and recording client progress and results
