About the Role
We are currently looking for an experienced Families Together Practitioner to join our team. Key duties will include:
- Engaging families through strengths based and culturally safe relationship building
- Delivering intensive family support through home visits and community outreach
- Undertaking holistic strengths, needs, and risk assessments
- Developing and reviewing family led case plans and safety plans
