Seeking an experienced Family Nurse Practitioner (FNP)
100% outpatient primary care practice
Monday - Friday, 8:00 AM - 5:00 PM
Minimal call responsibilities
Patient population is exclusively adults ages 65+
Average daily patient volume: 12-15
Value-based care model with a strong emphasis on quality outcomes and patient-centered care
Collaborative care team includes a full-time physician, nurse practitioners, medical assistants, and care coaches
Candidates must be fluent in Spanish, as the practice serves a large Spanish-speaking patient population
Minimum of two years of full-time experience practicing as a Nurse Practitioner in a primary care setting required (no new graduates)
