Our Youth Transition to Adulthood program is hiring a Transitions Worker focused on empowering youth. In this role, you will aid young adults in achieving housing stability, educational goals, and informed choices while addressing their mental health and addiction needs. Your contributions will help enhance their social networks and foster overall well-being.
Key Responsibilities:
• Assist youth in developing independent living skills
• Navigate support systems including mental health resources
• Build connections with natural supports
• Help locate and maintain safe housing solutions
• Coordinate life skills groups and recreational opportunities
