Home Health Aides- 25818

+ Within Nassau and Suffolk, NY 12123
+ Starting at $25/HR depending on Experience
+ Home Health Aides - 25818
Long Island Developmental Disabilities Service Office requests Home Health Aides to provide routine, personal healthcare service to developmentally disabled persons temporarily assigned to hospitals, skilled nursing facilities, rehab centers, etc. within Nassau and Suffolk counties as needed.
Requests for service will be submitted to the contractor solely by one of the following personnel assigned to Long Island DDSO:
+ DDSO RN Supervisor/Designee
+ Treatment Team Leader/Designee of the unit to which the service recipient is assigned.
(Long Island DDSO will provide the contractor with the names of the staff members listed above.)
The following is required of the Aide provided by the awarded contractor:
