Position Overview
The Home Care Program provides a broad range of services to assist people to safely remain in a community setting. Under the general supervision of the Manager Health Services and while demonstrating a commitment to the mission, vision and values of the WRHA, the Hospital Based Case Coordinator (HBCC) receives referrals and conducts assessments to determine eligibility for Home Care program supports. The HBCC, in collaboration with the client, family/carer and relevant interdisciplinary team members, develops, coordinates and evaluates the plan of care to support discharge from an acute care setting. In addition, the HBCC assists in the determination and planning if the client’s needs are best supported in a personal care home or alternate care environment.
