Job Description
Overview
The Licensed Practical Nurse, under the guidance and supervision of a Registered Nurse, administers routine services in caring for sub-acute, chronic and convalescent adult/geriatric residents. This care is rendered in accordance with established policies and procedures. To provide necessary nursing care for the residents of this facility in keeping with the Mission, Vision and Goals of Catholic Health Services of Long Island.

+ Practices within the LPN scope of practice
+ Identifies and reports change in resident status to RN in a timely manner
+ Performs and is knowledgeable of vital signs, medication administration, monitoring lab values, care of resident pre/post procedure , geriatric competency, hospice care, pain mgmt ., resident safety, restraints, isolation precautions/PPE, admission and discharge of patients/residents
