Cape Heritage Rehabilitation & Health Care Center in Sandwich is hiring an MDS Coordinator!
Shift is 8a-4p with weekly pay. Can be an LPN or RN. $42 to $48 an hour, depending on license. Cape Heritage is a 120-bed skilled nursing facility.
As the MDS Coordinator, you help plan, organize, and direct the MDS process, involving overseeing resident care plans through clinical assessment, review of residents' medical history, personal interviews, and completion of MDS reports.
Experience & Education:
Must possess, as a minimum, a nursing degree from an accredited school of nursing, college, or university.
Must possess an active and unencumbered license to practice as an LPN or RN in Massachusetts.
