Great opportunity in Sacramento, CA for a Nurse Practitioner seeing Post Transplant Care patients working for a private group.
- Family Nurse Practitioner (FNP) or Adult Nurse Practitioner (ANP)
- Minimum 2 yrs exp as NP preferably with transplant, nephrology, or complex chronic disease populations
- 40 hours a week - Mon-Fri 8a-5p
- Duties will include working in a clinic 3 days a week and 2 days a week doing comprehensive health work via telehealth for lab review and follow-ups
- Rare visits to home - maybe once a month if that would be required when necessary
- Clinic flow - patients get labs drawn in clinic, then see NP, then NP reviews labs 48 hours later, and then follows up with patients and nephrologists
- Program aims to proactively manage care - many patients have disengaged from nephrology follow-up despite being high risk
