The Nurse Case Manager (RN or LPN), based in Wisconsin and under the direction of the Director of Clinical Operations will manage and coordinate the health care for friars in Mt Calvary, Milwaukee, and Appleton. This position will serve as a resource to the guardians of these Capuchin communities as well as the Director of Clinical Operations. This role is a mix of clinical coordination, case management, and relationship-building—supporting holistic wellness while preserving dignity and independence.
Key Responsibilities
•Coordinate and manage healthcare plans for friars with acute and chronic conditions
•Develop, document, and monitor individualized plans of care
•Provide education and guidance on disease management and specialty care referrals
•Conduct routine health check-ins (at least every 6 months; more frequently as needed)
•Support hospital discharge planning and ongoing aftercare coordination
