Provide impactful primary care as a Nurse Practitioner on Sherbourne's Mobile Health Bus, addressing the healthcare needs of vulnerable populations in Toronto. This full-time role supports the delivery of essential health services while promoting well-being through community partnerships.
The Nurse Practitioner will collaborate with a multidisciplinary team to offer comprehensive care to individuals facing challenges like homelessness and substance use. You will advocate for client access to healthcare resources while adhering to the College of Nurses standards. In this role, you will have the opportunity to participate in community education and support initiatives.
Key Responsibilities:
• Assess and manage episodic client health needs
