Job Description:

• Conduct a comprehensive assessment of patients.
• Design and implement individualized treatment plans.
• Collaborate with families, physicians, and multidisciplinary teams to set therapy goals and monitor progress.
• Provide hands-on home exercise programs, positioning, and safe mobility strategies to patients/ caregivers.
• Maintain accurate documentation and contribute to progress reports and interdisciplinary meetings.
