Join Brightshores Health System as a full-time Outpatient Pharmacist in Owen Sound, specializing in medication therapy optimization. Collaborate with healthcare professionals to ensure safe, effective patient care.
As a key member in the Outpatient Pharmacy, you will report directly to the Manager of Pharmacy. Your role involves assessing therapeutic appropriateness of prescriber orders and leading Registered Pharmacy Technicians. In addition to resolving drug therapy issues, you will focus on enhancing patient outcomes during transitions of care.
Key Responsibilities:
• Assess prescriber orders for dosage and interactions
• Oversee Registered Pharmacy Technicians' work
• Support patients through care transitions
• Ensure adherence to safety standards in medication use
• Collaborate within an interdisciplinary health team
Requirements:
• Licensed or eligible for Ontario College of Pharmacists
• Experience in medication reconciliation and problem-solving
