Drive efficiency in patient transitions as a Patient Flow Specialist at our acute care facility. This role emphasizes collaboration with nursing units and physicians to ensure timely movement of patients.
Qualified candidates will be Registered Nurses, preferably with a four-year degree and 3-5 years of experience in acute care settings. You will need to demonstrate skills in patient flow coordination and a strong ability to resolve barriers affecting discharges. The position entails daily monitoring and reporting on patient flow to senior leadership.
