Summary:
The primary care pharmacist will initiate and develop comprehensive medication management services within the primary care setting. This position is an integral member of the health care team, working collaboratively with other primary care providers, medical assistants, care managers, nurses, social workers and family members to ensure the delivery of quality, efficient, and cost-effective healthcare.
Responsibilities:
+ Manages individual patients and patient populations that have complex and/or multiple medication needs for acute and chronic disease states.
