You will report directly to the Manager of EMR Clinical Applications and focus on integrating Mackenzie Health with local primary care strategies. Your responsibilities will include gathering requirements and supporting the development of tools that manage chronic diseases and improve workflows within community care settings.
Key Responsibilities:
• Collaborate with WYROHT to develop integrated care pathways
• Build strong relationships with community partners
• Lead initiatives to streamline referral processes
• Provide ongoing system support and troubleshooting
• Enhance system functionality and maintain documentation
