Responsibilities
- Complete admission resident nutritional assessment and determine level of nutrition risk as per Home nutrition profile/assessment and nutrition risk screening policy.
- Provide further nutritional assessment, documentation and development of nutritional care plans and follow-up for identified residents at high nutritional risk and some at moderate risk.
- Participate in resident care planning regarding nutrition issues for new admission residents and those at high nutrition risk.
- Calculate food intake records for identified residents eating poorly and make recommendations accordingly.
- Review and approve changes made by the food service supervisor to the generic regular menu, therapeutic menu and nourishment menu for spring/summer and fall/winter cycle menus.
