The Bassano Health Centre is an acute and continuing care facility with 4 acute care beds and 8 continuing care beds. As a member of the CoAct team, you will assess, design, and implement continuing care services for a caseload of clients in this setting. You will play a key role in resident care planning, collaborating with interdisciplinary teams, residents, and families to help residents achieve their goals and enhance their quality of life. The role is suited for candidates with a strong interest in caring for elderly clients and a skill set in patient‑ and family‑centered care.
