Overview
Integrated Home Care uses a strengths‑based, client‑centered case management framework. We develop care plans that support clients and families to identify needs, focus on strengths, set achievable health and self‑care goals, and provide direct nursing care. In collaboration with the client/family and interdisciplinary team, we ensure services are administered and modified on an ongoing basis, based on the client’s response to interventions, incorporating stewardship into service authorization. The Home Care Manager oversees coordination and quality care for clients in the community.
