Responsibilities

• Assess, plan, implement, and evaluate individualized care to meet the residents' care needs. Ensure resident care plan is initiated in collaboration with other members of the multidisciplinary team.
• Assume responsibility for medication management including administration, appropriate recording, and safekeeping of all drugs.
• Assist with scheduling of resident's care conferences and participate in conferences where applicable.
• In conjunction with the RHA Manager, participate in discharge planning, consultation, and/or transfer of residents.
