Resident Case Management:
+ Initial and periodic resident evaluation.
+ Orientation of residents and families to the daily routine and follow up to ensure smooth transition
+ Assisting each resident to maintain family and comm unity ties a nd to develop new ones.
+ Encouraging resident participation in facility and community activities.
+ Establishing linkage with and arranging for services from public and private sources for income, health, mental health, and social services.
