Be a pivotal part of Community Health Centres of Northumberland as a full-time Rural Outreach Clinic Nurse Practitioner. Engage with rural populations through comprehensive health services and outreach strategies.
This role focuses on providing client-centered care across various health domains, including mental health, women’s health, and chronic disease management. You will enhance service access by transitioning unattached patients to continuous primary care and collaborating with community resources. The Nurse Practitioner will work independently and in teams to promote health and improve patient outcomes in rural settings.
