Social Worker - Children's Disability Team
Our Children's Disability Team works with disabled children and their families, assessing their need for social care services and supporting Child in Need and Child Protection plans for children under Care Proceedings, Children We Care For and Leaving Care adults. This role offers the opportunity to maximize participation and reflect children's rights in service delivery by acting on the views of children and young people.
Responsibilities
- Assess need and provide focused work to help parents and children make changes, recording and evaluating outcomes and liaising with other agencies and voluntary organisations.
- Serve as the allocated worker for disabled children identified by assessment and for disabled children who are looked after.
