In this role at the Urgent and Primary Care Centre, you will participate in team-based care, managing a patient panel and providing both longitudinal and urgent care services. Your work will directly impact the health of diverse patients and families, particularly the medically complex and at-risk groups. Experience in mental health and addiction treatment is considered an asset.
