Southeast Advocates is seeking a Transition youth with disabilities transitioning from high school to post-secondary education, employment, or other community pathways. The position works directly with students within school settings and in the community. Regular travel to schools in Weyburn, Estevan and surrounding areas is required. Approved travel expenses will be reimbursed.
Duties: Provide one-on-one and group support to youth with disabilities. Develop and implement individualized transition plans in collaboration with students, families, school staff and community partners. Support career exploration, employment readiness, life skills and goal setting. Liaise with employers, post-secondary institutions and service providers.
